A 17 year old student with fever, chills and pain abdomen

Chief complaint-
A 17 year old student, a resident of ogode presented with chief complaints of fever with chills and pain abdomen

History of present illness-
The patient was apparently asymptomatic 4 days and developed fever with chills, during evenings and pain abdomen since 4 days.
He visited a RMP doctor who prescribed him Paracetamol and gave him an injection.
His fever was relieved on medication but he developed severe pain abdomen the next day.
He visited a local hospital(nekrekal) where he was diagnosed with dengue and decreased platelet count
He eventually came to KIMS as they had health card of KIMS and got admitted here on Saturday 
He was infused a unit of blood on Sunday,
He then complained of 4 episodes of vomiting(non-bilious and non-projectile), black colored stools, brownish urine and tightness of abdomen the same day

Past illness-
No history of HTN, Diabetes, Asthma

Blood transfusion History-
One unit on Sunday

Family History-
His father is a known case of Diabetes 

Personal History-
Diet: mixed
Appetite: normal
Sleep: adequate 
Bowel: regular
Bladder: regular
Addictions: none

Surgical history- no H/o any surgeries 

General Examination-
Patient is concious, coherent and cooperative
Moderately built and moderately nourished
Pallor- absent
No icterus
No cyanosis
No Clubbing of fingers
No Generalised lymphadenopathy
No pedal edema 

Vitals-
Temperature: 98.2⁰F
Pulse rate: 72 bpm
Blood pressure: 110/60 mm of Hg
Respiratory rate: 20 cpm
SpO2: 98% at room temp

Systemic Examination-
CVS: 
S1, S2 heard
No murmurs

Investigations-
Fever chart-
ECG-
